The framework,
applied to your situation.
Three distinct coaching tracks — for parents, for leaders, and for organizations. Each one applies the Formation Framework to a specific context with a specific goal.
Parent Formation Coaching
You have read the book, taken the assessment, or heard the talk. You know where your teenager is on the map. Now you need help knowing what to do about it — with your specific child, in your specific family, in this specific season.
Parent Formation Coaching is a structured series of sessions walking you through the Formation Framework as it applies to your teenager’s current status, your family culture, and what you are trying to build together. The goal is not to fix your teenager. It is to help you become the kind of parent whose presence creates the conditions for genuine formation.
Leadership Humility Coaching
Built around the LHP-5 — the Leadership Humility Profile — this track is for leaders who want to understand how their own formation history is shaping the way they lead. The LHP-5 assesses five dimensions of leadership humility and maps them against the Formation Framework to show where a leader’s identity is owned versus performed.
This is not executive coaching in the traditional sense. It is formation work applied to leadership — examining the interior conditions that produce the exterior behaviors that either build or erode the organizations and teams a leader is responsible for.
Collective Identity Consulting
Organizations form identities the same way individuals do. They explore or foreclose. They commit from the inside or perform for external audiences. The Formation Framework applies to teams, institutions, and organizational cultures — and the diagnostic is just as revealing at the collective level as it is at the individual one.
Collective Identity Consulting applies the framework to organizations navigating transition, identity drift, culture erosion, or leadership succession. It begins with a diagnostic conversation and moves toward a structured engagement built around the specific formation challenge the organization is facing.
All three coaching tracks are limited in availability. Dr. Usita works with a small number of clients at a time to ensure the depth of engagement each track requires. Inquiries are handled personally and typically receive a response within two business days.

Before any coaching conversation, the best first step is the Identity Status Assessment. It takes five minutes and gives both of you a clear starting point.
